Tout le monde connaît les « avécés ». Ils sont causés soit par une hémorragie, soit par une thrombose (un caillot), cas le plus fréquents.
Depuis plusieurs années la thrombolyse, par utilisation d’anticoagulants in-situ, avait diminué de façon évidente le nombre et l’importance des séquelles neurologiques de la thrombose cérébrale. Mais elle restait limitée aux artères cérébrales de gros calibre, et à condition d’être mise en route de façon très précoce : 4 h 1/2 après l’accident. Ce qui en limite forcément l’utilisation.
Une nouvelle étape vient d’être franchie avec la thromboaspiration, qui consiste à aspirer le caillot responsable en reliant la sonde sur le vide, par l’intermédiaire d’un ou plusieurs catéthers très fins et très souples : le système Penumbra, qui peut être utilisé seul ou avec anticoagulants.
Plusieurs avantages : moins de risques, et surtout plus de cas traités par allongement du délai d’intervention d’une heure et demie : 6 h au lieu de 4 h 30.
Mais évidemment ce « système Penumbra » n’est guère encore disponible dans beaucoup d’hôpitaux. Cela va venir….
